This roundtable will explore how pharmacy-enabled neighbourhood teams can deliver integrated, patient-centred care for people at risk of cardiovascular, renal, metabolic conditions, and obesity. The discussion will focus on building pharmacy capacity, including independent prescribing, diagnostic access, data visibility, and patient coaching, while aligning time and incentives with meaningful outcomes.
 

The session will examine medicines optimisation, standardising protocols for initiation, titration, and deprescribing, and ensuring the right medicines reach the right patients at the right time. It will also highlight digital and data enablers for shared care planning, real-time collaboration, and coordinated care, while considering how national policy and incentives can support or hinder progress.
 

A population-health perspective will underpin the discussion, strengthening pharmacists’ leadership, enhancing pharmacy technicians’ roles, and driving improvements in long-term condition management in line with the NHS 10-Year Plan.

Ghulam Haydar  

National Senior Policy Lead, Pharmacy, NHS England​

Ghulam Haydar is a Senior Policy Lead within the Pharmacy team at NHS England. He is an experienced Pharmacist Independent Prescriber in Primary Care. Ghulam is currently a national lead on the Community Pharmacy Independent Prescribing Pathfinder Programme and is also involved in Pharmacy First. He has previously undertaken roles as a Clinical Advisor at the General Pharmaceutical Council on Pharmacist Independent Prescribing, a training lead for foundation year pharmacists in General Practice and a clinical skills training practitioner for foundation year pharmacists in secondary care. Ghulam is currently a PhD candidate at the University of the West of England, undertaking research into Community Pharmacy Independent Prescribing.

Stephen Riley

Stephen Riley is a senior healthcare leader and registered pharmacist with over 18 years of experience across the NHS, private, and voluntary sectors. He currently serves as Deputy Regional Chief Pharmacist at NHS England Northwest and is a member of the GPhC Fitness to Practice Committee. Riley has held national policy roles, chaired multiple pharmacy committees, and worked with organizations like the PDA, Pharmacist Support, NICE, and the Royal Pharmaceutical Society. He is a strong advocate for professional development, inclusion, and diversity in healthcare.

​David brings over eight years of experience delivering strategic healthcare initiatives across a range of long-term conditions. As Associate Director for Medicines Optimisation and Clinical Lead for Long Term Conditions, he leads system-wide programmes that bridge innovation and implementation to drive meaningful change in communities and reduce health inequalities.​

Alistair Gray

Chief Pharmacy Information Officer, NW Regional Digital Pharmacy & Medicines Lead​

Alistair has worked at East Lancashire Hospitals since 2002, having previously worked for Boots. He innovated the Refer-to-Pharmacy and the Dedicated Ward Pharmacy services. As Chief Pharmacy Information Officer he manages the Trust’s electronic prescribing and medicines administration system which includes working with multiple health professionals developing effective Transfer of Care workflows and Fluid Stewardship protocols. He is NHSE’s Northwest Regional Digital Pharmacy & Medicines Lead. He is a member of the Royal Pharmaceutical Society’s Digital Expert Advisory Group. He co-authors the Clinical Pharmacy Pocket Companion and the Injectable Drugs Guide. He has a band called The Al-Chemists and loves Formula 1.
